ABSENCES: Madison Meadows School asks that parents or guardians call the school attendance clerk, Mrs. Giny Peoples, prior to 8:30 a.m. each day that a student is absent.The school does not condone absences for reasons other than illness, religious reasons, medical appointment(s), or bereavement. If the school has not been notified of an absence, upon return to school, a note from a parent/guardian or physician is required explaining the absence. Unexcused absences will result in a detention. Students with excessive, excused absences may be asked to bring a doctor's excuse for absences, or will be referred to the City of Phoenix AIM (Attendance is Mandatory) program. . the information contained herein, feel free to contact me. KEEPING YOUR BUS RIDING PRIVILEGES All students are entitled to a safe and harassment free bus ride to and from school. The following rules will help you keep your bus riding privileges: 1. The bus driver is the authority on the school bus. SEVERE CLAUSE: ANY act that is potentially harmful or dangerous to any person or property will result in IMMEDIATE referral to the school principal. MAKE-UP WORK AFTER ABSENCE: If the student is absent for one day, make-up work will be provided upon his/her return. If the student is absent more than one day, work will be provided by teachers as requested by parents. If work is not requested during a student's absence, the student will be allowed the same number of days absent for the completion of all assignments. Work may be requested by calling the nurse's office before 10:00 a.m. for assignments. Work will be available after 3:30 p.m. in the office. Work requested after 10:00 a.m. will be available the following day. Requested assignments are to be picked up from the homework box located in the main office. Please note that it is the student's responsibility to obtain and make up all assignments missed during an absence. Fifth Grade Supply List 2007—2008 The list below includes materials needed by our incoming Fifth grade students. Please plan to purchase these items before the beginning of the 2007-2008 academic year. Not listed are a dictionary, a thesaurus, and a public library cards which should have at home for personal use. Please anticipate replenishing supplies at the start of the second semester; a notice will be sent home at that time. In an effort to alleviate the amount of material in a student’s backpack, items marked with an asterisk (*) will be collected by your child’s homeroom teacher. General items ALL Students will need: One dozen –sharpened #2 pencils (for student) One Public Library card 5th Grade Mathematics One – 2” Three-Ring Binder (NO Trappers!) 5th Grade Science: One – Duo-tang folder 5th Grade Social Studies: One – Duo-tang folder 5th Grade Spanish: Two – two pocket folders with prongs 5th Grade Language Arts: 2 inch, Three-Ring Binder (NO TRAPPERS!) Math and Language Arts Notebooks will be used in class each day. Students are expected to arrive on the first day with the supplies; notebooks should not be put together at home. At the start of the academic year, teachers will organize and label sections with their classes. These three-ring binders will be used AND KEPT in class, so as not to weigh down backpacks. Notebooks will be streamlined occasionally with teacher supervision and will be collected and assessed on a regular basis. Please NO sharpies, mechanical pencils, or Trappers. The list below includes materials needed by our incoming Sixth grade students. Please plan to purchase these items before the beginning of the 2007-2008 academic year. Please note that the listed materials are a minimum; additional supplies may be requested by a particular teacher. We wish to emphasize that NO TRAPPERS will be accepted. We are hoping that the weight of the backpacks will be kept to a minimum.
